This app is for legal aid organizations looking for standard templates that request for a client’s medical records from the Social Security Administration to aid in the appeal process for Social Security disability benefits. The form included in this workflow is the Letter to the Social Security Administration template. It is important to note that these templates include Minnesota states statutes. Unless you are a legal aid organization in Minnesota, these templates must be updated to reflect your state’s statutes. This is only possible thanks to the generous funding by Legal Services Corporation through the Technology Innovation Grant.
You can find the rest of SMRLS’ free SSI/SSDI workflows on Gavel’s Marketplace: SSA Letter to Client, Medical Records Request no Affidavit, Medical Records Request with Affidavit, and Medical Records Fee Waiver.
