Tips for filling out form UD-125

When completing form UD-125, pay special attention to these less obvious sections:

Item 1

Defendant's Responsibility

Make sure you, as the defendant, are the one filling out this form. It's specifically designed for defendants in an unlawful detainer case seeking to stop eviction due to COVID-19 rental debt. If you are not the defendant, consult with the actual defendant to complete the form correctly.
Item 2

Time Period Selection

Only check the boxes in 2(a) that correspond to the time period during which your rent was due and has not been paid. It’s crucial to accurately select the correct rental periods covered by your government assistance for the court to assess your situation properly.
Item 3b

Missing Approval Documents

If you do not have a physical copy of the approval from the government rental assistance program, make sure to accurately provide the property address, application number, and, if known, the name of the program. This information is vital for the court to verify your claim and determine the applicability of rental assistance.
